Cat Poop Problem

My cat is 13YO and lately I am finding poop dropping over the house. I thought she wasn't using her box, but today it looked like some of flat yellow thing was hanging out of her butt. The last time I took her to the vet she was fine. What might be causing this problem? Also she attempts to cover poop etc once out of box and she ends up scratching side of box.

she may be having trouble going and so it is not coming out properly and then drops out later somewhere else.

If she is constipated this may be uncomfortable for her and so could be causing her to become anxious causing the strange behaviors.
