sick cat has lost lots of weight

our cat got outside and came home after about a week she hen began to lose lots of wieght on a reguler basis she is very itchy and scrates so bad that she has big wounds on her neck and head we have used flea meds and it dosent appear to be fleas we were told to go ahead and deworm her so we did she has started to gain back a little wieght but not much the back of her legs have no hair on them and the skin is really red and scaby she acts as if he dosent get enough to eat she eats all the time. she is still acting as herself physcally but please help

I think it would be a good idea to take her to th vets to have her skin looked at and perhaps some tests to be carried out. the weight loss may be connected as she may be anxious due to her itchiness.
